Care Coordination

Integra has a Nurse Care Manager dedicated to working with each skilled nursing facility partner. This nurse will make certain that all of your care needs are met throughout your stay, as well as ensure there is a smooth and successful return to your community.

*Facilities marked with an asterisk participate in the Medicare Shared Savings Program three-day waiver. Our ACO is approved to send Medicare patients for skilled nursing facility or rehabilitation care even if they haven’t stayed in a hospital for three days first, which is usually a requirement in Medicare. For Medicare patients to qualify for this benefit, we’d have to decide that they need skilled nursing facility care and meet certain other eligibility requirements. The skilled nursing facility three-day rule waiver applies only to eligible Medicare patients who get care from one of our skilled nursing facility partners.
